I am not contracted with any insurance carriers as an in-network provider. All fees are due at the time of service. Upon request, a superbill can be provided to the client in order to submit to your insurance company. If you are unsure if your insurance will provide reimbursement for a portion of out-of-network care, call them and ask them about your out-of-network benefits.
I maintain a strict 24 hour cancellation policy. If you need to reschedule or cancel an appointment, you can use the patient portal or send me a direct email. Do not respond to the automated email and text reminders from Simple Practice because I will not receive them. Any missed appointment or cancellation made with less than 24 hours notice from the scheduled appointment time will be subject to a cancellation fee. This is necessary because a time commitment is made to you and is held exclusively for you.
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
